Student Enrollment (New)

New Student Enrollment Checklist:

New Student Enrollment is completed by appointment only. Please call the Guidance Office at 708-342-5821 to schedule an appointment, and possible placement testing once you have collected all required documents.

REQUIRED STUDENT INFORMATION - Students must be withdrawn from their current school before registering.
______ Illinois State Health Physical (9th Grade or more recent; out of state students must have an IL Physical)
______ Immunizations/Shots Records
______ Birth Certificate/Passport (passports are for International students only)
______ ISBE Student Transfer Form (only required if the student is transferring from an IL high school)
______ Discipline Records or Good Standing Letter (a letter stating student is not currently suspended or expelled; for those students transferring from a parochial school or from out of state)
______ Withdrawal Grades/Grades in Progress
______ Unofficial Transcript (upperclassmen and second semester freshmen)
______ IEP/Special Education Records/Section 504 Plan (if applicable)
______ 8th Grade Report Card (incoming Freshmen)
______ Standardized Test Scores (ACT, SAT, PSAT, MAP)(students without these scores may be tested prior to registration)
______ Divorce/Custody/Guardianship Papers/906 Placement Form - copies of first page, custody page, and signature page (if applicable)

REQUIRED PROOFS OF RESIDENCY FROM PARENT/HOMEOWNER

(1) of the following:
______ Most recent Property Tax Bill & Proof of Payment (canceled check, proof from cookcountrytreasurer.com,
                     and/or mortgage statement with proof of payment listed for property taxes)
______ Current Lease (with parent/guardian name & address) & Proof of Payment - Lease must be valid on the
                     first day of the school year
______ Current Mortgage Statement
______ Closing Statement for the recent purchase of residence (within the past 60 days)

(3) of the following (must include parent name & address & dated within the last 60 days):
______ Drivers License or State ID ______ Vehicle Registration ______ Most Recent Cable Bill
______ Current Public Aid card with name & address
______ Current homeowners/renters insurance policy & proof of payment
______ Current Gas Bill
______ Current Electric Bill
______ Current Water Bill

8th Grade transition for students currently residing in D230 boundaries, and attending the public school within the boundaries:

As District 230 continues to streamline processes based on feedback from families, the district will again use the CLEAR public database to automatically verify student address information. This process is an efficient way to assure that students enrolled in our schools are residents of the District 230 community. The annual residency verification process safeguards district resources for taxpayers and is convenient for families.

CLEAR uses parent's name and address to crosscheck the information against public electronic databases such as utility companies. This process will be completed in early May.
 
If your residency is verified through CLEAR, there will be no need for you to submit any additional paperwork to certify that you live within district boundaries. 
 
If your residency is not verified through CLEAR, the school will contact you to inform you of the documents required which can be submitted online using the link below or in-person at the school by appointment.
